  • Complete Student Application Form 
  • Registration Fee and Tuition Payment
  • Record of Immunizations
  • Social Security Card (copy)
  • Birth Certificate (copy)
  • Release Academic Records Form
  • Teacher Evaluation of Student Behavior
  • Financial Contract Agreement
